The sports waiver must be completed by all registered players as follows,
1) Print the form, add the signature in the small box at the top right hand corner, complete the player info section address etc.
Date the form bottom left hand corner
Add signature to bottom right corner and print your name
Have a witness sign and print name bottom left corner.
Signatures must be handwritten not typed then scan the form back to oassc2@gmail.com.
No Photos of the waiver will be accepted.
2) The insurance company will accept waiver forms that are signed from a cell phone app.
3) Print waiver form complete as instructed above and mail by Canada Post to OASSC 58 Madill Drive, Mono, On L9W6G4
